Benzodiazepines, a class of medications used to treat anxiety or insomnia, comprise another group that has been linked with cognitive difficulties. Although these medications are truly a blessing for some individuals immobilized by anxiety, their use can be accompanied by sedation and mental slowing. Why cholesterol drugs might affect memory
ONE DAY IN 1999, Duane Graveline, then a 68-year-old former NASA astronaut, returned home from his morning walk in Merritt Island, Fla., and could not remember where he was. His wife stepped outside, and he greeted her as a stranger. When Gravelines memory returned some six hours later in the hospital, he racked his brain to figure out what might have caused this terrifying bout of amnesia. Only one thing came to mind: he had recently started taking the statin drug Lipitor.
Cholesterol-lowering statins such as Lipitor, Crestor and Zocor are the most widely prescribed medications in the world, and they are credited with saving the lives of many heart disease patients. But recently a small number of users have voiced concerns that the drugs elicit unexpected cognitive side effects, such as memory loss, fuzzy thinking and learning difficulties. Hundreds of people have registered complaints with MedWatch, the U.S. Food and Drug Administrations adverse drug reaction database, but few studies have been done and the results are inconclusive. Nevertheless, many experts are starting to believe that a small percentage of the population is at risk, and they are calling for increased public awareness of the possible cognitive side effects of statinssymptoms that may be misdiagnosed as dementia in the aging patients who take them.

After more than 20 years as a judge, Madeline came to me distraught and miserable.
I dont know whats wrong with me, but I cant remember anything, she confided in my office. I sit in court all day, listening to testimony, and by that evening Ive forgotten all of it and have to review the entire days conversations. And even then, its a struggle to recall what I just read! Risks Should Be Carefully Considered
To assess the strength of anticholinergic drugs and how often the participants took them, the team looked at available information about prescriptions over a period of 10 years.
However, they note that this is an observational study, so they cannot confirm whether the drugs are directly responsible for the increased risk of dementia.
The researchers add that doctors may have prescribed some of these drugs to their patients precisely for the treatment of very early dementia symptoms.
Nevertheless, Prof. Coupland argues that the study adds further evidence of the potential risks associated with strong anticholinergic drugs, particularly antidepressants, bladder antimuscarinic drugs, anti-Parkinsons drugs, and epilepsy drugs.
The risks of this type of medication should be carefully considered by healthcare professionals alongside the benefits when the drugs are prescribed and alternative treatments should be considered where possible.

These findings also highlight the importance of carrying out regular medication reviews.
We found a greater risk for people diagnosed with dementia before the age of 80, which indicates that anticholinergic drugs should be prescribed with caution in middle-aged people as well as in older people, she concludes.

Common Causes Of Forgetfulness
Memory slips are aggravating, frustrating, and sometimes worrisome. When they happen more than they should, they can trigger fears of looming dementia or Alzheimers disease. But there are some treatable causes of forgetfulness. Here are six common ones.
Lack of sleep. Not getting enough sleep is perhaps the greatest unappreciated cause of forgetfulness. Too little restful sleep can also lead to mood changes and anxiety, which in turn contribute to problems with memory.
Medications. Tranquilizers, antidepressants, some blood pressure drugs, and other medications can affect memory, usually by causing sedation or confusion. A Fluke In The Results And Areas For Future Research
One result that left researchers scratching their heads was the fact that the patients who took drugs that do not cross the blood-brain barrier had better outcomes on attention compared with those who took drugs that cross to the brain. Although cardiovascular disease does negatively affect attention, it is not a sign of dementia. Nation and Jean Ho, PhD, a postdoctoral scholar at the UCI MIND, are pursuing new studies for a better understanding of those results.
For now though, this new research could help pave the way for different prescribing practices that take into account other diseases linked to hypertension.
Alzheimers has been built as a disease of the brain in which nothing else has anything to do with it, says Constantino Iadecola, PhD, the director of the Feil Family Brain and Mind Research Institute at Weill Cornell Medicine, who was not involved in the study. Vascular damage to the brain is caused mainly by high blood pressure. It’s also a major contributor to Alzheimer’s disease. So the message to the community is that if you treat high blood pressure, you also treat Alzheimer’s disease.
Iadecola advises patients to check your blood pressure regularly to get treated as soon as possible. He also advises Black patients in particular who have a tendency toward hypertension to treat high blood pressure now in any shape or form.
Midlife High Blood Pressurea Risk Factor For Cognitive Decline
After decades of observational research, theres general agreement that high blood pressure in middle age is a risk factor for later-life cognitive decline, including overall cognition, memory, and processing speed.
Much of the data comes from studies of large groups of adults who were followed for many years. For example, the NIA-supported Honolulu-Asia Aging Study measured cognition in 3,734 Japanese-American men in Hawaii, with an average age of 78, whod had their blood pressure measured more than 20 years earlier. Researchers found that for every 10-mmHg increase in systolic blood pressure, there was a 9 percent increase in risk for poor cognitive function.
Similarly, the Atherosclerosis Risk in Communities neurocognitive study followed more than 13,000 white and African-American adults, starting at age 48 to 67, for 20 years. In later life, performance on cognitive tests of short-term memory, executive function, and processing speed fell for all participants. Those with high blood pressure or prehypertension at the start of the study scored worse than those with normal blood pressure. Participants who used drugs to treat their high blood pressure scored better than those with untreated hypertension, but not as well as those with normal blood pressure.

Lisinopril And Liver Damage
As a drug class, ACE inhibitors have been associated with rare cases of acute liver injury, specifically cholestatic liver injury. However, lisinopril is different from other ACE inhibitors in two ways:
- Apparent liver injury is more common with lisinopril than other ACE inhibitors.
- The type of liver injury lisinopril causes is hepatocellular, rather than cholestatic.
Both types of liver injury are serious, but hepatocellular liver injury involves liver cell death and can ultimately be fatal. According to the drug labels for Prinivil and Zestril, patients who develop jaundice or drug-induced hepatitis should stop taking the drug and seek help from a medical expert right away.
Many of the lawsuits filed against companies that make drugs containing lisinopril have focused on liver damage. Plaintiffs argue that drugmakers like Merck and AstraZeneca should have done more to warn patients about this potential adverse event.
Icipants And Study Design
These analyses use longitudinal data from the Cardiovascular Health Study, a prospective multi-center, population-based cohort study of cardiovascular risk factors in 5,888 community-dwelling older adults. An ancillary study was conducted to evaluate the incidence and prevalence of dementia in a subset of the cohort who had brain magnetic resonance imaging between 1991 and 1994., Participants were evaluated at baseline with clinical assessments including physical, cognitive, and functional assessments as well as laboratory testing. Medication, function, cognitive, physical exam, and medical history data were updated at annual participant visits. For this analysis, participants determined to have prevalent dementia at the time of the MRI were excluded. In order to limit confounding by indication, we also restricted our analyses to participants with treated HTN defined as self-reported HTN and taking an antihypertensive medication. Because ACE inhibitors are also commonly used for congestive heart failure treatment and because heart failure may impair cognition, we further excluded patients with heart failure at baseline. After applying these entry and exclusion criterion, 1054 participants formed our study population .
