How Trauma Affects The Brain

A traumatic incident can cause a great deal of stress in both the short term and the long term. That stress response can have an impact on different areas of the brain, such as the hippocampus, amygdala and prefrontal cortex. In fact, those areas of the brain can change in shape and volume, and experience diminished function.

Not coincidentally, these are areas of the brain that are strongly associated with memory function. The prefrontal cortex helps process working memory, the information that we need to remember on an everyday basis. The hippocampus is also a major memory center in the brain. The left hippocampus focuses on memorizing facts and recognition, while the right hippocampus is associated with spatial memory. The hippocampus also gives us a way to learn by comparing past memories with present experiences. And the amygdala processes fear-based memories if you ever burned your hand on a stove once, you remember not to touch the hot surface again because the memory is processed and stored by the amygdala. The amygdala is also believed to help with the formation of long-term memory. Trauma-based memory loss, therefore, can easily occur when the trauma creates stress that negatively affects the brain.

When it comes to trauma and memory loss, there are different types of trauma that can cause temporary or permanent problems.

Mild cognitive impairment is a common, age-linked condition that is often an early sign of Alzheimer’s disease. Its cardinal symptom is forgetfulness or impairment of short-term memory.

Numerous drugs have been shown to produce mild cognitive impairment . They may create or aggravate Alzheimer’s-type symptoms.

Most of the drugs that cause MCI have a property called “anti-cholinergic.” They inhibit activity of the neurotransmitter acetylcholine, which plays a critical role in memory and cognitive function.

Here’s the problem: only a few of these drugs are officially classified as anti-cholinergic. The official anti-cholinergic drugs are mostly used for relieving intestinal cramps or bladder irritability and are labeled “anti-spasmodic.” They’re at the top of the list below.

But there are 17 additional types of drugs used for many other purposes that may also have anti-cholinergic effects. The list includes commonly used drugs like antihistamines, acid blockers and antidepressants. Unfortunately, many doctors and pharmacists are unaware of the anti-cholinergic properties of these medications.

Several published studies have also shown that people taking drugs with hidden anti-cholinergic effects are at increased risk for MCI.

How Anticholinergics Affect The Aging Brain And Body

Anticholinergics are drugs that block acetylcholine, a key neurotransmitter in the body. This leads to lower brain function, which people often experience as drowsiness.

Sometimes that sedation is why people take the drugs, and a little sleepiness might sound benign. But when the brain is older, or otherwise vulnerable, these drugs can be problematic.

In fact, these literally have the opposite effect of the drugs often used to treat Alzheimers and other dementias. Donepezil, rivastigmine, and galantamine , for example, are designed to increase acetylcholine by blocking the brain enzyme that breaks it down.

Research has linked anticholinergic drugs to increased risk of Alzheimers disease, and also to hospitalizations in older adults. And the American Geriatrics Society has warned about them for years anticholinergics are definitely on the Beers List of medications older adults should avoid or use with caution.

Acetylcholine is a neurotransmitter used in many other parts of the body, including the eyes, mouth, bowels, and bladder. So anticholinergic drugs commonly cause side-effects such as dry eyes, dry mouth, and constipation.

These drugs are in everything from allergy medicines to muscle relaxants to painkillers. They are in many over-the-counter medications, and they are often prescribed for a variety of common health complaints.

Opioids also impair both short and long-term memory by inhibiting our perception of pain within the central nervous system and dulling our reaction to it. The Yale School of Medicine examined individuals who overdosed onopiates and suffered amnesia as a result. What they found was that there was swelling in the cerebellum and hippocampus.

In general, brain swelling can result in memory loss. Drug-induced brain swelling makes permanent memory loss and the ability to form memories overall even more likely. Opioid abuse can result in other cognitive disabilities down the line without medical intervention.

Effect Of Acute And Chronic Administration Of Tramadol On R Index

Intraperitoneal administration of 20 mg/kg single dose of tramadol 90 min before the T1 trial significantly decreased the R index in the T2 trial compared to the control. Daily i.p injection of tramadol at 20 mg/kg for 21 consecutive days reduced the R index significantly in comparison to the control group.

Although tramadol could diminish the R index both in acute or in chronic administration compared to their respective controls, the single dose injection of tramadol reduced the R index in the T2 trial more noticeably than when it chronically administered in repeated daily dose for 21 days .

Suicidal Thoughts And Self

Additional discussions should be had around the risk of developing or progressing in suicidal ideation and/or thoughts of self-harm while taking gabapentin. If you, or a loved one, are beginning gabapentin therapy, it is often recommended to seek out additional support with a psychotherapist or psychologist while on the medication to help safeguard the individual taking gabapentin and monitor for any suicidal thoughts or self-harm.

These side effects can be serious yet treatable and manageable with proper care and support by family, loved ones and mental health professionals.

Dietary Approaches For Mci

There is research to suggest that diet plays a role in dementia. However, its not yet clear what particular diet will prevent MCI progressing.

I personally agree with the common-sense approach that a diet that is good for overall health is likely to be good for brain health.

One of my favorite clinical nutrition trials is titled the Cocoa, Cognition, and Aging Study. In 2012 the researchers of a study in which they found that giving people with MCI a daily cocoa drink led to improved cognitive function and insulin metabolism eight weeks later.

Now, we still dont know if daily cocoa would change the likelihood of having dementia a few years later. But its a promising result, and you can learn about more such promising foods in Dr. Gregers book.

How Could Confounding Affect The Study Results

With this new study of tramadol, confounding is a real concern. For example, for a person who has both kidney disease and arthritis, doctors may prescribe tramadol rather than naproxen because the latter may worsen kidney disease. Yet kidney disease could increase the risk of other health problems, including a higher rate of death, which could then be attributed to the tramadol. In other words, the very reason your doctor chose tramadol could make this medication appear riskier than it really is.

The authors of the study acknowledge this possibility and took measures to limit it. In fact, many studies try to avoid these sorts of errors, but they are impossible to avoid completely.

The National Institute on Alcohol Abuse and Alcoholism defines two types of blackouts that occur as a result of alcohol.

The first is called a partial blackout, and this can happen with just a relatively small amount of alcohol. You may forget things like the names of people or information that you would ordinarily be able to easily recall.
