There is a great deal of literature on the effects of various forms of stress on the biology and behavior of animals and humans. Interestingly, changes in the regulation of endocrine systems, changes in learning and memory, changes in the histology of certain parts of the brain, changes in behavior and other changes show strong resemblances between stressed animals and depressed humans. This appears to validate the use of animal models to explain stress and antidepressant mechanisms in humans. Therefore, stress-induced brain changes in animal models of depression may validly represent the brain changes in depressed humans, and antidepressant-induced changes in animal models may validly represent antidepressant mechanisms in depressed humans.

The Problem With Normalizing Period Pain

As for the fine print, SSRIs also have well-documented side effects, including decreased sexual desire, weight gain, insomnia, headaches and nausea. And its debatable how well SSRIs work for anxiety in particular; some experts argue they can do more harm than good.

Why Antidepressant Withdrawal

Antidepressants work by altering the levels of neurotransmitters chemical messengers that attach to receptors on neurons throughout the body and influence their activity. Neurons eventually adapt to the current level of neurotransmitters, and symptoms that range from mild to distressing may arise if the level changes too much too fast for example, because you’ve suddenly stopped taking your antidepressant. They’re generally not medically dangerous but may be uncomfortable.

Among the newer antidepressants, those that influence the serotonin system selective serotonin reuptake inhibitors and serotonin-norepinephrine reuptake inhibitors are associated with a number of withdrawal symptoms, often called antidepressant or SRI discontinuation syndrome. Stopping antidepressants such as bupropion that do not affect serotonin systems dopamine and norepinephrine reuptake inhibitors seems less troublesome over all, although some patients develop extreme irritability.

Having discontinuation symptoms doesn’t mean you’re addicted to your antidepressant. A person who is addicted craves the drug and often needs increasingly higher doses. Few people who take antidepressants develop a craving or feel a need to increase the dose.

When To Talk With A Doctor

Individuals should speak with their doctor if they are experiencing unwanted side effects of antidepressant use. Their doctor may recommend switching to another drug or taking other medications to reduce adverse reactions.

Those who wish to come off their antidepressants should ask their doctor how to do so safely with medical supervision.

People who are experiencing discontinuation symptoms after coming off medication should make an appointment with their doctor. If these symptoms are severe, seek prompt treatment.

Antidepressants

An SSRI is a type of antidepressant. Examples include fluoxetine , paroxetine , and sertraline . These drugs make a neurotransmitter called serotonin more available in the brain. Experts believe this helps reduce symptoms of depression and anxiety and regulate mood.

SNRIs are another type of antidepressant. Examples include duloxetine and venlafaxine . These drugs work on two neurotransmitters: serotonin and norepinephrine.

These two classes of antidepressants are groundbreaking. Older antidepressants had many hard-to-tolerate side effects, including severe fatigue, dry mouth and eyes, and difficulty urinating. In contrast, SSRIs and SNRIs are generally well tolerated. Like all drugs, though, they do have side effects, including effects on sexual function, which can be difficult to talk about, but are important to tell your physician. Each drug is a little different, and an experienced psychiatrist can help people choose the best option for them.

We encourage patients to think broadly about treating depression. Treating depression with medicine can be very effective. Talk therapy, including cognitive behavioral therapy, also has been shown to be very helpful, as have exercise and social activities. Think about treatment for depression like a menu at a restaurant where you can choose foods from more than one column. Often, a combination of medicine and nondrug treatment maximizes the benefit of both.

If youre thinking about stopping antidepressants, you should go step-by-step, and consider the following:

Take your time.;You may be tempted to stop taking antidepressants as soon as your symptoms ease, but depression can return if you quit too soon. Clinicians generally recommend staying on the medication for six to nine months before considering going off it. If youve had three or more recurrences of depression, make that at least two years.

Talk to your clinician about the benefits and risks of antidepressants in your particular situation, and work with her or him in deciding whether to stop using them. Before discontinuing, you should feel confident that youre functioning well, that your life circumstances are stable, and that you can cope with any negative thoughts that might emerge. Dont try to quit while youre under stress or undergoing a significant change in your life, such as a new job or an illness.

Make a plan.;Going off an antidepressant usually involves reducing your dose in increments, allowing two to six weeks between dose reductions. Your clinician can instruct you in tapering your dose and prescribe the appropriate dosage pills for making the change. The schedule will depend on which antidepressant youre taking, how long youve been on it, your current dose, and any symptoms you had during previous medication changes. Its also a good idea to keep a mood calendar on which you record your mood on a daily basis.

Youll Cut Back Slowly On The Meds

To help you avoid as many unpleasant side effects as possible, your doctor will carefully taper you off of the medication. The weaning protocol for each drug is different, Dr. Binder says, but typically adjustments will be made two to three weeks apart to give your doctor a good sense of how your symptoms change as you taper.

Stopping suddenly can be very difficult to tolerate, Dr. Binder says. Never stop cold turkey.

Doctors usually accomplish this by either cutting your dose or cutting back on how frequently you take it. Either way, its like landing a plane, says Robert Valuck, Ph.D., a professor in the department of clinical pharmacy at the University of Colorado. You dont want to drop from 35,000 feet to the runway; you want a nice, slow glide path. Some people can handle a steeper glide path, says Valuck, but both approaches gradually reduce the dose in your system so that your body can adjust in baby steps.

Types Of Antidepressant Medications

There are several classes of antidepressants; within each class there are many individual medications. While all antidepressants work well overall, no drug or type of drug works equally well for everyone who takes it. You may be advised to try more than one type of antidepressant or to use a combination of antidepressants to seek relief from your distress.

The different types of antidepressants are listed in the order in which they are most commonly prescribed. Medications are referred to in two ways: by their generic name and by their brand or trade names. Brand names available in Canada appear in brackets.

SSRIs

This group of drugs, including fluoxetine , paroxetine , fluvoxamine , citalopram , escitalopram and sertraline , is usually the first choice for treatment of depression and anxiety disorders. These medications are known to have milder side-effects than some other antidepressants. Buspirone is similar to SSRIs and has been found to help with anxiety but not depression.

Common side-effects include nausea, vomiting, diarrhea, weight gain, dry mouth, headaches, anxiety, sedation and a decrease in sexual desire and response. This group of drugs may also cause a jittery or restless feeling and sleep difficulties, such as problems falling asleep, waking in the night, vivid dreams or nightmares.

Blood Sugar & Diabetes

Several studies have noted what appears to be a link between antidepressant use and problems with blood sugar regulation, including type 2;diabetes.

A systematic review published in a 2013 edition of the journal Diabetes Care examined this relationship to get a better feel for what’s going on. They looked at 22 studies, including a couple with more than 4,000 participants. Here’s a look at some of the findings that prompted the review:

  • Antidepressants may worsen blood sugar control because they can cause significant weight gain.
  • SSRIs and Pamelor reportedly worsen blood sugar control in people with diabetes.
  • Tricyclic antidepressants cause hyperglycemia in humans.
  • In mice, tricyclic antidepressants cause a condition called hyperinsulinemia, in which the blood contains too much insulin relative to the amount of sugar.

The aim of the review was to determine whether antidepressants raise the risk of diabetes in people who didn’t have it when they started on the medications. They concluded that some antidepressants affect blood sugar regulation and that the drugs could be a risk factor for diabetes. However, the larger and more recent studies they looked at suggested that the risk was small.

If you have never had an episode of depression before then it is reasonable to begin to taper off the medication gradually after you have been free from depression for nine months. If you have had three or more episodes of depression then the likelihood of having another episode at some point in the future is roughly 90% and therefore, for many people, long-term treatment with an antidepressant makes sense. If you have had one or two previous episodes of depression your risk of recurrence is somewhere between 30% and 90%. Factors such as a family history of depression, current stress, etc. will influence your risk of recurrence.

    Do Antidepressants Cause A Chemical Imbalance Likely

    Im not sure when pharmaceutical companies started marketing that a chemical imbalance is the cause of depression. Either way, this is a great marketing ploy that really gets people to believe that if they are feeling depressed, it must be a result of a natural chemical imbalance. In reality, most people do not likely have major problems with levels of various neurotransmitters.

    However, the most popular way to treat depression is the quick fix method taking a drug before trying other alternatives. What most drugs do is prevent the reuptake of the neurotransmitter serotonin. The neurotransmitter serotonin is involved in influencing: sleep, aggression, impulsivity, mood, and anxiety levels. When serotonin levels increase, it may make some people drowsy and result in feelings of pleasant relaxation.

    Although SSRIs work for awhile, most people either develop a tolerance and come off of them OR they cannot cope with the detrimental side effects. When a person withdraws from an SSRI, a new problem is created a chemical imbalance. Serotonin levels do not dramatically get restored overnight it can take months to correct the imbalance that was created by SSRIs.
