From Conception To Age Three: An Outline Of Early Brain Development

First Trimester

The development of the brain begins in the first few weeks after conception. Most of the structural features of the brain appear during the embryonic period ; these structures then continue to grow and develop during the fetal period .19,20

The first key event of brain development is the formation of the neural tube. About two weeks after conception, the neural plate, a layer of specialized cells in the embryo, begins to slowly fold over onto itself, eventually forming a tube-shaped structure. The tube gradually closes as the edges of the plate fuse together; this process is usually complete by four weeks after conception. The neural tube continues to change, eventually becoming the brain and spinal cord.20,21

About seven weeks after conception the first neurons and synapses begin to develop in the spinal cord. These early neural connections allow the fetus to make its first movements, which can be detected by ultrasound and MRI even though in most cases the mother cannot feel them. These movements, in turn, provide the brain with sensory input that spurs on its development. More coordinated movements develop over the next several weeks.22

Second Trimester

Early in the second trimester, gyri and sulci begin to appear on the brains surface; by the end of this trimester, this process is almost complete. The cerebral cortex is growing in thickness and complexity and synapse formation in this area is beginning.20,21,23

During labor and delivery, it is important that healthcare providers appropriately monitor mother and infant.  Any signs of fetal distress should be taken seriously and should be immediately acted upon.  That may mean more consistent monitoring of fetal vital signs, or ordering a c-section to prevent oxygen deprivation.

During and after delivery, it is also important that healthcare providers take measures to prevent head trauma.  During delivery, head trauma may occur as a result of pressure, use of delivery assistance tools, or a doctor who uses too much force.  After delivery, healthcare providers must monitor the infant for signs of fetal distress, jaundice, stroke, or infection.  They must also be careful to properly handle and support the infants head to prevent injuries.

At birth, the average babys brain is about a quarter of the size of the average adult brain. Incredibly, it doubles in size in the first year. It keeps growing to about 80% of adult size by age 3 and 90% nearly full grown by age 5.

The brain is the command center of the human body.   • Brain activity levels provide long term prognosis. A year after injury, 44% of patients whose brains were reactive to stimuli were able to take care of themselves for at least eight hours, compared to 14% of those whose brains did not respond.

Advancements in measuring brain activity are timely in response to a growing number of TBI and coma injuries.

When Are Brain Scans Performed On Babies With Suspected Brain Injury

Multiple brain scans are essential because newborn brain injury evolves over hours, weeks and days. In addition, an injured brain may make the baby susceptible to later development of brain bleeds, hydrocephalus and other conditions that require prompt treatment.

Often, brain injury in a newborn baby occurs during or close to the time of birth. When a brain injury is suspected, doctors will look for signs and symptoms of global brain dysfunction, also known as neonatal encephalopathy. Some forms of brain damage can be seen at 12 24 hours after birth, and edema is usually evident at this time as well. An MRI can usually show moderate to severe forms of HIE within 12 to 24 hours of birth.

When a premature baby has periventricular brain damage, the damage will usually be evident on the CUS when the baby is 24 hours old, and cystic changes will typically be seen after approximately one to three weeks. An MRI is best for assessing the extent of white matter brain damage and it can usually show brain injury by 1 day of age. Once the initial brain damage is established in a baby, serial scans should be done so the medical team can evaluate the changing brain injury. Sometimes radiologists miss evidence of brain damage on a scan or the damage may not be evident. Thus, it is not uncommon for scans to be performed every day when a baby shows signs of a brain injury.

The new work focused on the default mode networka collection of regions that are active when the brain is at rest, when someone is daydreaming or letting their mind wander, not concentrating on a specific task. Fetal brains are in default mode for much of the time, but it is not very well known how this network develops.

This is one of the first papers to take individual fetuses and look at the naturally developing default mode network in the human fetal brain, says Vinay Pai, Ph.D., director of the Division of Health Informatics Technologies at NIBIB. Since movement of the baby or the mother is no longer an issue, It allows you to look at more natural developmental stages.

The technique can also be used to compare differences in brain development in premature and full-term babies; the effects of alcohol, drug use, or stress during pregnancy; or if there are any prenatal differences in babies that go on to develop neurodevelopmental disorders like autism. And it is not restricted to imaging the brain; Studholme also plans to study the placenta and how its development influences the brain.
